This includes Slow-Paced Rehabilitation (Restorative Care), Medically-Complex Care and Palliative Care beds.<\/p>\n<p><strong>Applying for Admission<\/strong><br \/>\nPatients can be admitted from Hospital or from the community and can be referred by any healthcare professional who identifies that a patient would benefit from the program. The patient or a member of their circle of care (family physician, home care workers, family members, nurse practitioners and clinics) may contact our Patient Flow Coordinator to discuss the case and determine if the patient would be a good fit for our program.<\/p>\n<p>Would you like to discuss a potential case or ask questions about our programs?
