St. Joseph’s Continuing Care Centre is on a continuous journey of quality improvement — employing new evidence-based practices and processes so that we can continue to provide the highest quality care for our patients and their families.

Our Quality Improvement Plan (QIP) is one of the tools we use to track our performance in a selection of high-priority areas, including patient safety and satisfaction, and access to care. This plan represents our ongoing commitment to achieve and lead in system-wide quality and safety best practices that improve patient experiences and outcomes, and also provide accountability for reaching the highest possible quality and value for health care in Ontario.

Each year, as part of the Excellent Care for All Act (2010) we publicly post our Quality Improvement Plan and submit it to the provincial government so they can track improvement performance across the system. We believe it is important for the population we serve across the province to see how we are meeting their needs by delivering excellent care.
